Staying on Zepbound After Goal Weight: What the Trials Show

Reached your goal weight on Zepbound (tirzepatide)? Here's what the SURMOUNT-4 trial found about continuing versus stopping, what the label says about doses, and what to track and ask your prescriber.

You hit your goal weight. Now what?

This is one of the most common questions in the Zepbound community. Whether to stay on, change your dose, or stop is a decision for you and your prescriber. This guide covers what the trials show, what the label says, and what to track and ask so that conversation goes well.

What SURMOUNT-4 Found

SURMOUNT-4 (Aronne et al., JAMA 2024) is the main trial on what happens after weight loss on tirzepatide:

  • Everyone took tirzepatide for 36 weeks, working up to their maximum tolerated dose (10 mg or 15 mg).
  • They were then randomly assigned to keep taking it or switch to placebo for the next 52 weeks.
  • People switched to placebo gained 14.0% of their body weight over those 52 weeks.
  • People who continued tirzepatide lost a further 5.5%.

The takeaway: in this trial, stopping tirzepatide led to substantial regain on average, while continuing it held and extended the loss.

What the Label Says About Doses

The Zepbound label starts at 2.5 mg, which is a starting dose for tolerability, and lists 5 mg, 10 mg and 15 mg as maintenance doses. Doses go up in 2.5 mg steps after at least 4 weeks on the current dose.

The label and the trials don't give a protocol for lowering your dose after you reach goal weight, and lower-dose maintenance hasn't been well studied. If you're wondering whether a different dose, or stopping, makes sense for you, that's a question for your prescriber. Your prescriber decides your dose and pace. Don't change your dose or the spacing between injections on your own.

What to Track at Goal Weight

Whatever you and your prescriber decide, tracking a few signals each week gives you both something concrete to go on:

What to track What to watch for
Weight trend (7-day average) A steady upward drift rather than day-to-day swings
Food noise Constant thoughts about food, or cravings coming back
Hunger Feeling hungry soon after meals, or portions creeping up
Strength Lifts or everyday tasks getting harder

If any of these move the wrong way, bring it to your prescriber rather than adjusting your medication yourself.

Habits That Help You Hold Your Weight

Medication decisions belong to your prescriber, but these are yours:

  • Resistance training a few times a week to protect muscle
  • A daily protein target (if you have kidney disease, ask your clinician what's right for you)
  • Regular weigh-ins so you catch drift early
  • Consistent meals so you aren't relying only on medication-driven appetite suppression

Questions to Bring to Your Prescriber

  • "Given the SURMOUNT-4 results, what do you recommend for me now that I'm at goal: staying on, a different dose, or stopping?"
  • "If we change anything, how will we monitor it, and what would make us change course?"
  • "What weight change or return of food noise should prompt me to contact you?"
  • "How should we plan if my coverage or cost changes?"
  • "Does anything about my other health conditions or medications affect this decision?"

If you're not sure how to start, you could say:

"I've reached my goal weight on [dose]. I'd like a plan to keep the weight off long-term. Can we talk about what the SURMOUNT-4 results mean for me?"

Bottom Line

The strongest trial evidence, SURMOUNT-4, shows that continuing tirzepatide helps people keep weight off and that stopping leads to substantial regain on average. There's no established protocol for lowering the dose at goal weight, so any change is a decision for you and your prescriber. Track your weight trend, food noise, hunger and strength so you both have real data to work with.
